Step 2: Schedule your appointment.

Step 3: Complete new patient forms. These can be downloaded, printed, completed at home and brought with you for your first appointment. If you prefer, the forms can be completed when you come to the office for your first appointment. 

Step 4: Arrive 15 minutes early for your first appointment.
